Intracoronary brachytherapy: past, present, and future.

چکیده

The purpose of intracoronary brachytherapy is to reduce the incidence of lesion recurrence (restenosis) after percutaneous coronary intervention. Its rationale is based on the concept that restenosis is a process of benign tissue proliferation and that locally applied radiation can prevent or attenuate the magnitude of this response. After encouraging results from animal models,1,2 small observational and randomized clinical trials were initiated to demonstrate the feasibility and safety of this unique application of radiation.3–5 Positive findings led to larger randomized trials that demonstrated a powerful treatment effect.6,7 In patients presenting with restenosis within a previously implanted stent, the likelihood of repeat restenosis was reduced by 50%. Results of brachytherapy administered to patients undergoing balloon angioplasty alone or at the time of stent implantation have been inconsistent. At this time, brachytherapy is considered to be a standard treatment alternative for selected patients with in-stent restenosis, and it is performed as a component of routine practice in interventional cardiology.

Overview of Intracoronary Brachytherapy for in-Stent Restenosis of a Drug-Eluting Stent

Percutaneous coronary intervention with stenting is considered recently as the most common procedure for the treatment of symptomatic coronary. The article reviewed 41 studies published during 1997-2019 on intracoronary brachytherapy for in-stent restenosis of a drug-eluting stent. Intracoronary radiation therapy was finally confirmed in the setting of in-stent restenosis using as adjunctive th...

Modeling and dose calculations of a pure beta emitting 32P coated stent for intracoronary brachytherapy by Monte Carlo code

Background: Recently, different investigators have studied the possibility of radiation therapy in restenosis prevention and have shown promising results. In this study a unique radioactive source for intra vascular brachytherapy (IVBT) was investigated. The two-dimensional dose distribution in water for a 32P IVBT stent has been calculated. The pure beta emitter source 32P has been co...

Intracoronary Radiation Therapy (IRT) for In-Stent Restenosis

The efficacy of beta and gamma emitters in reducing clinical and angiographic restenosis in patients with in-stent restenosis has been confirmed by a number of studies. This review is intended to give an overview of the individual trials, summarize pertinent lessons that have been learned and give insight into the future of intracoronary radiation. The encouraging results from the clinical tria...

Brachytherapy: here today, gone tomorrow?

B y the late 1990s intracoronary brachytherapy had become the gold standard therapy for the treatment of in-stent restenosis (ISR) in bare metal stents. Despite this, the ‘‘uptake’’ of this treatment by the interventional community worldwide, and certainly in the UK, was low. In 2005, drug eluting stents (DES) have entered the interventional armamentarium. The potential for DES to lower the pri...

Intracoronary brachytherapy for the prevention and management of restenosis.

Patients who undergo percutaneous transluminal coronary angioplasty (PTCA) for coronary artery disease are at risk for recurrence at the site of the procedure. Placement of stents as an adjunct to PTCA is one strategy to reduce recurrence. The use of various techniques to apply intracoronary radiation through local application of catheters or placement of radioactive stents (also known as intra...
